You Settle Into Your New Home
Once you move in, our team will continue listening, checking in and adjusting your support as your needs or goals change.
We Explore Your
Housing Options
We introduce you to Vera Living to assist you in ensuring your plan is right for this style of support. Vera Living will communicate with you and your Support Co-ordinator to apply for the property you have chosen.
You Choose Your Support Team
You will work with us to choose your support workers from our team who suit your needs, preferences and personality. Your support workers will receive training specific to your care requirements.
Ongoing Exceptional Care
Exceptional Care for You will continue to provide your supports as per your personal support plan, developed in consultation with you and tailored to your individual needs, goals and preferences.
You Visit the Home
We can visit the property with you so you can explore the space, ask questions and decide whether it feels right for you. We’ll be there right with you.
We Prepare for a Smooth Transition
Before moving day, we’ll work with you, your family, support coordinator and other health professionals to make sure the right supports are in place. Your supports will be delivered as per the hours in your plan. A monthly flat rate will be determined for food and utilities.
