for NDIS Participants
Support for independant living
(SIL)
As much or as little help as your participant needs, including meal preparation, cleaning, running errands, personal care and whatever else they need to live as autonomously as they can
Social and community participation
Social support when, where, and however your participant pleases — quiet social chats, help with hobbies, getting to and from community activities, and fun
Aids for Daily Living
(ADL)
Sensitive, respectful help with bathing, grooming and dressing. High care needs met with 24/7 complex support, with companionship, kindness, and joy
Support workers recruited to match participant preferences
Taking exceptional care to match support workers who 'click' with participants and become their companions and advocates. That’s in our DNA. We’ll match your participants with support workers from our large team of trained disability support workers. We’re also happy to recruit the exact right support worker to meet your clients' specific needs.
Clinical oversight - consistent, compassionate, free
Your participants' care team is led by the Registered Nurse, who is the first person they meet at their initial intake visit. This is a really important relationship. Your participant’s nurse is involved in all care planning and reviews.
Our clinical oversight never wavers or loses its focus.

Because we believe your client is entitled to ongoing access to an RN's expertise, experience and insight.
​
And we make sure they get that support whenever and however they need it. This clinical oversight is at no additional cost.
Here's how it works:
Free initial clinical consultations and onboarding visits by our RNs
​As a new participant, your client meets with one of our RNs. So, they feel seen, heard, and safe, knowing we understand their needs from the beginning.
Care plans constantly monitored
Your participant's care plan is developed and regularly reviewed by our RNs: to

Ensure you’re kept up to date with issues of concern,

Educate our support workers about changing needs, and (with consent)
-
Inform their loved ones (with consent) of changes in health, function, and overall well-being.
24/7 on-call nursing support
For your participants AND for you. Our RNs are one phone call away, day or night

Simple Referrals, Genuine Partnership, Impeccable Care
A clear, responsive and compassionate referral process that respects the trust you’ve placed in us, makes your role easier, and ensures your participants feel safe, heard, and genuinely cared for from day one.
We respond to your referral promptly. As soon as an initial review confirms we can meet your participant’s care needs and budget requirements, we schedule an initial visit.
Our intake process
1. First Visit – Your participant meets one of our Registered Nurses
Comfort and confidence begin with familiarity.

In this visit, the RN:
​​​

Introduces who we are and how we work

Delivers and discusses:
-
a comprehensive information pack
-
a personalised introductory video featuring our office team and support workers, so participants can see the faces behind the support

Gathers important information about the participant’s needs and preferences
-
Begins to understand their goals, routines and support expectations
After this visit, our nurse follows up with the participant to check in and discuss next steps.
2. Second Visit – Care Planning and signing the Service Agreement
If your participant wishes to proceed and we are confident we can provide the right support, our nurse returns to:

Develop a personalised care plan together with your participant

Discuss the Service Agreement so your participant feels confident to sign it
-
Share some staff profiles and photos, and propose some introductions so your participant can get a sense of who may be supporting them
Wherever possible, we introduce our care staff before services commence.
​
Care usually begins within one to two days of this visit.
3. Finding the match and building our relationship
When you and your participant entrust their care to us, we go way beyond providing (exceptional) services. We build treasured relationships.
​
At the heart of this relationship is our constant care and attention to ensuring staff matches are the right fit.
​
When a new staff member begins caring for your participant, our nurse follows up with both of them to check that the match feels comfortable and aligned.
​
If a new team member is introduced later, we repeat the process.
4. Regular check-ins and care reviews
Consistent communication, transparency and genuine collaboration build trust.

Our Care Services Manager personally:

Makes contact after onboarding

Checks in at around three-to-four-weeks
-
Follows up again at six weeks
Ongoing care plan reviews, regular check-ins, requests for feedback and updates to Support Coordinators are integral parts of your participant’s journey.

Our Care Services Manager and CEO, Di, are available during business hours to take calls and enquiries directly.

Once a participant is onboarded, we provide 24-hour phone access for urgent matters. You and your participant are never without support, especially when something goes awry.
​
Our communication with Support Coordinators is ongoing and proactive — via phone and email. You’ll always be updated on progress, changes or concerns.
​
If any issues arise, we raised directly and promptly with the Support Coordinator.​
